Role Summary:
This position is integral to achieving corporate goals by applying expertise in Long-Term Services and Supports (LTSS) to deliver effective Case Management.

Key Responsibilities:
- Conduct intake, screening, and referral processes, along with assessments and reassessments.
- Develop and implement support plans, ensuring ongoing case management and monitoring of members' health and welfare.
- Maintain thorough documentation of interactions and case management activities in the designated system.
- Cultivate resources and manage case closures appropriately.
- Uphold confidentiality and respect privacy as outlined in the UnitedHealth Group Employee Handbook.
- Execute monitoring contacts as required, ensuring regular communication with members based on their conditions.
- Perform face-to-face meetings with members at least biannually or more frequently as needed.
- Update assessment tools and care plans to reflect any changes in members' conditions or services.
- Complete additional assessments as necessary and monitor the delivery of services outlined in the Support Plan.
- Attend training sessions provided by the department and participate in agency-sponsored professional development.
- Ensure compliance with the requirements of the CMA Contract Agreement, delivering accurate and timely work.
- Report any suspicions of mistreatment or abuse to the appropriate authorities immediately.
- May undertake special projects and assist with system assessments as required.
- This role necessitates travel; reliable transportation is essential for visiting various locations.

Qualifications:
Required:
- A Bachelor’s degree in a relevant field such as human services, nursing, social work, psychology, or public health.
- A minimum of 5 years of relevant experience in LTSS or a combination of education and experience.
- At least 1 year of proficiency with MS Office applications including Word, Excel, and Outlook.
- Access to dependable transportation and willingness to travel up to 75% within the assigned area.
- Residency in the designated service areas.

Preferred:
- Experience as a caseworker or case manager within the LTSS population may substitute for educational requirements on a year-for-year basis.

The hourly wage for this position is competitive and based on various factors including local labor markets, education, and experience. In addition to salary, UnitedHealth Group offers a comprehensive benefits package, including incentive programs and retirement contributions.
