Nurse Practitioner Opportunity - Critical Care Transition

About the Role:
TotalMed is seeking an experienced nurse practitioner to join our team in the critical care transition area. As a key member of our interdisciplinary team, you will be responsible for providing high-quality patient care to patients transitioning from intensive care to general medical-surgical units.

Key Responsibilities:
• Conduct thorough assessments of patients' physical and emotional needs in the Step Down Unit setting.
• Develop and implement individualized treatment plans in collaboration with the interdisciplinary team.
• Provide education and support to patients and their families regarding post-intensive care recovery, medication management, and self-care techniques.

Requirements:
• Current NP license
• BLS (Other Certifications as required by facility)
• Two years of recent experience in critical care or Step Down Unit settings
• Excellent communication and interpersonal skills.
