Position Overview

The MDS Coordinator plays a crucial role in ensuring the timely and precise execution of the RAI process and care management from the point of admission through to discharge, adhering to company policies, as well as Federal, State, and Certification guidelines.

Key Responsibilities

  • Collaborate with the Interdisciplinary Team to evaluate resident needs and establish schedules for MDS assessments and care plan reviews in compliance with regulatory requirements.
  • Guide the Interdisciplinary team in determining the necessity of additional MDS assessments based on clinical criteria outlined in the latest RAI User's Manual.
  • Facilitate daily stand-up meetings to review resident care and set assessment reference dates.
  • Ensure compliance with federal and state regulations concerning the RAI process.
  • Oversee MDS and care plan documentation for all residents, ensuring that medical records support MDS coding.
  • Maintain an up-to-date MDS status for assigned residents according to regulatory standards.
  • Accurately enter resident information into the appropriate computerized MDS systems.
  • Conduct thorough coding of the MDS using data from medical record reviews, observations, and interviews with staff, residents, and families.
  • Participate in interdisciplinary team meetings and quality assurance sessions to gather information and update records.
  • Assist the Director of Nursing or designee in identifying significant changes and ensuring that MDS and care plans reflect these changes.
  • Prepare schedules and notifications for resident care planning conferences and assist in communicating outcomes to relevant parties.
  • Continuously enhance knowledge related to data entry and technological advancements.
  • Submit required documentation electronically to state databases and other entities in accordance with company policies.
  • Ensure the completion and accuracy of final MDS submissions to the appropriate government agencies.
  • Provide guidance and training to staff regarding the RAI Process, PPS Medicare, Medicaid, and related clinical systems.
  • Maintain confidentiality of sensitive information.
  • Other duties may be assigned as necessary.

Qualifications

  • Graduate of an accredited Registered Nurse program and licensed in the state of practice.
  • Preferred: Minimum of 2 years of nursing experience in a Skilled Nursing Facility.
  • Strong understanding of Case-Mix, Federal Medicare PPS, and Medicaid reimbursement processes.
  • Familiarity with the Quality Indicator process and OBRA regulations.
  • Knowledge of care planning processes.
  • Experience with MDS 3.0 is preferred.
