Clinical Assessment Advisor Home Health Weekend Baylor

CenterWell Home Health is a leading provider of home care services, and we're seeking a skilled Clinical Assessment Advisor to join our team. As a Clinical Assessment Advisor, you will play a critical role in developing and overseeing patient care plans, ensuring optimal clinical outcomes and continuous patient improvement.
Key Responsibilities:
- Develop and implement patient care plans in collaboration with physicians and healthcare professionals.
- Conduct ongoing assessments of patient needs, identifying areas for improvement and developing strategies for continuous care.
- Coordinate with multidisciplinary teams to ensure seamless patient care, including home health aides, nurses, and other healthcare professionals.
- Collaborate with physicians and referral sources to obtain relevant clinical information and optimize patient care.
- Ensure timely completion of care plans, assessments, and other documentation.
- Participate in quality improvement initiatives and contribute to the development of new policies and procedures.
Requirements:
- Bachelor of Science degree in Nursing (BSN) or equivalent experience.
- At least 2 years of clinical home care experience, with a minimum of 1 year in a clinical management role.
- Current CPR certification and excellent communication and interpersonal skills.
- Knowledge of state and federal home health agency regulations and compliance standards.
- Ability to read, write, and speak fluent English.
