Hospice Registered Nurse (RN)

**Job Summary:**

The Advanced Hospice seeks a skilled Hospice Registered Nurse (RN) to join our team. This role plays a vital part in planning, organizing, and directing home care services with a focus on community health education and experience.

Key Responsibilities:
  1. Initial Assessment and Care Planning:
  • Conduct an initial assessment of patient and family needs.
  • Determine home care requirements and develop a comprehensive plan of care.
Patient Care:
  • Provide physical assessments and histories of current and previous illnesses.
  • Regularly re-evaluate patient nursing needs.
  • Initiate and revise care plans as necessary.
Communication:
  • Prepare clinical notes and update primary physicians.
  • Communicate with community health professionals to coordinate care plans.
Additional Duties:
  • Participate in on-call duties.
  • E nsure equipment and services are available.
  • Instruct and supervise home health aides.
**Requirements:**
  • Current licensure to practice professional nursing in the state.
  • Graduate of a National League for Nursing accredited school of nursing.
  • Maintain current CPR certification.
  • Licensed driver with insured vehicle in good working order.
  • Minimum two years' experience in public health or home care nursing preferred.
**Essential Skills:**
  • Self-directed with minimal supervision required.
  • Excellent observation, problem-solving, verbal, and written communication skills.
  • Able to organize and prioritize workload independently.
  • Responsible for supervising home health aides.
