**Job Summary and Estimated Salary**
$90,000 - $110,000 per year

Key Responsibilities:
- Coordinate patient admissions, transfers, and consults into accepting facilities from referring hospitals, clinics, and physician offices.
- Systematically process patient acuity and make expert-level clinical decisions to facilitate patient flow through the continuum of care.
- Use critical thinking skills to recommend optimal patient outcomes based on clinical information, patient/physician preferences, and facility capabilities.

Required Skills and Qualifications:
- 5 years of relevant work experience as a registered nurse.
- Graduate of an approved discipline-specific program.
- Strong leadership, communication, and problem-solving skills.
