About the Role

As a Nurse Practitioner with Alteas Health, you'll work part-time, two days per week in senior care facilities in a centralized region, providing pulmonary care to underserved populations. For those open to additional hours, there is an option to expand your schedule by traveling to the Greater Rockford and Chicagoland areas to provide coverage, as needed. As part of our team, you'll collaborate with experienced leaders in Pulmonary services while growing your career in an environment dedicated to mentoring and development.

Responsibilities
  • Analyze and interpret patients' histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses.
  • Diagnose or treat acute health care problems such as illnesses, infections, or injuries.
  • Recommends diagnostic or therapeutic interventions.
  • Order, perform, or interpret the results of diagnostic tests.
  • Maintain complete and detailed records of patients' health care plans and prognoses.
  • Prescribe medications.
  • Educate patients about self-management of acute or chronic illnesses.
  • Complete full patient consenting process.
  • Engage & coordinate with necessary internal and external clinical staff for continuity of care and best practice care coordination.
Requirements
  • Current and valid NP/PA licensure in IL.
  • Reliable transportation.
  • At least 6 months of NP/PA experience.
Preferred Qualifications
  • 1+ years of Nursing Home experience.
  • Specialization in Pulmonary.
