Job Title: Registered Nurse - Home Health

Kaiser Permanente is seeking a skilled and compassionate Registered Nurse to join our Home Health team in Redmond, Washington.

About the Role:

As a Registered Nurse in our Home Health program, you will provide comprehensive skilled nursing care and services to patients in their homes. You will work collaboratively with an interdisciplinary team to develop and implement individualized care plans that address the unique needs of each patient.

Responsibilities:
  • Conduct thorough assessments of patients' physical and emotional needs
  • Develop and implement individualized care plans that address patients' needs and goals
  • Provide skilled nursing interventions, including wound care, medication management, and education
  • Collaborate with patients, families, and healthcare providers to ensure seamless care transitions
  • Document patient progress and outcomes in a timely and accurate manner
Requirements:
  • Minimum two years of experience as a Registered Nurse in medical-surgical, home health, or hospice
  • Associates degree in Nursing from an accredited program
  • Registered Nurse License (Washington) required at hire
  • Basic Life Support certification required at hire
Preferred Qualifications:
  • Bachelor's degree in Nursing or a related healthcare field
  • Experience with home care systems and software
