Job Summary

The Licensed Medical Provider in this role works in an acute withdrawal management setting as well as in a residential treatment environment, caring for patients suffering from substance use disorders.

The provider works as part of a larger addiction medicine team, including experienced physicians, advance practice providers, PMHNPs, as well as RNs, LPNs, and substance use counselors.

The provider performs admission physicals, formulating and documenting a patient-centered plan of care for both management of withdrawal and initiation of maintenance medications to manage substance use disorders.

The provider also addresses other medical issues that arise in detox and residential clients as needed; participates in coordination of care with other team members internally, as well as with external providers including primary care providers, specialists, and the Emergency Department, when indicated.

This role will spend an average of approximately 30-40 hours per week, in a 3-4 day work week.

An average day consists of 4-8 hours of direct patient care and coordination, with 24 hour on-call access to provide admission orders for evening intakes and to triage occasional issues as they arise.

Essential Functions
  • Screens referrals for potential admission to addiction medicine services;
  • Conducts histories and physicals on patients within 24 hours of inpatient admission and within 72 hours of outpatient visit, and documents assessment and plan in electronic medical record (EHR);
  • Reviews nursing assessment done at inpatient admission and issues standing orders;
  • Issues any additional procedure orders for continuation of medications or initiation of medications needed to address ongoing medical conditions as well as lab orders;
  • Orders necessary labs and review results with patients;
  • Orders and interprets urine toxicology screens, and accesses the Oregon Prescription Drug Monitoring Database when necessary;
  • Provides counseling and education on medication options for the treatment of various substance use disorders;
  • Provides education regarding options for and rationale behind healthy SUD recovery and medical concepts related to addiction;
  • Formulates and initiates treatment plan with patient's informed consent and participation; Titrates or adjusts medications as needed;
  • In the inpatient environment, reviews CIWA/COWS/Vitals results (conducted by Nursing staff) to determine if patient's withdrawal symptoms have stabilized sufficiently for discharge on a day-to-day basis;
  • Administers medications for SUD treatment via subcutaneous/intramuscular injections, using standard practices including clean technique (training provided);
  • Uses motivational interviewing and active listening to support clients recovering from drug/alcohol dependence;
  • Addresses other general health concerns as they arise (to include contraceptive counseling, STI counseling and treatment, routine psychiatric care, emergent/temporary management of issues such as hypertension and diabetes, and treatment of chronic viral infections) and documents assessments and medical care in EHR, or refers to primary care, specialty care, or the emergency room as needed.

Qualifications
  • Medical Degree (MD, DO, PA, NP) from accredited university required;
  • Experience in addiction, behavioral health and dual-diagnosis care and treatment required;
  • For individuals in recovery from substance-use disorder, must have maintained continuous recovery for the previous two years at the time of hire;
  • Valid license to practice medicine in Oregon;
  • Current ALS/CPR Certification required;
  • Must maintain a valid Oregon Driver License or ability to obtain one upon hire, and be insurable under the organization's auto liability coverage policy (minimum 21 years of age and with no Type A violations in the past 3 years, or three (3) or more Type B violations).
Preferred Qualifications
  • Experience working with diverse, multi-cultural communities preferred;
  • Bilingual in English/Spanish a plus;
  • Must have demonstrated competency or ability to attain competency for each of the following within a reasonable period:

Patience, empathy, understanding of and awareness of working with individuals with mental illness, substance use disorders, and intellectual and developmental disabilities;

Strong client engagement skills;

High level of ability and comfort providing extensive outreach and community-based nursing care in a multitude of settings;

Proven success in collaborating with co-workers and a multi-disciplinary team to provide and advocate for excellent patient care;

Ability to work effectively and respectfully in a diverse, multi-cultural environment;

Proficiency in MS Office 365 (Word, Excel, Outlook), EHR systems, databases, virtual meeting platforms, internet, and ability to learn new or updated software;

Strong interpersonal and customer service skills, and demonstrated compassion towards individuals with mental illness and substance abuse disorders;

Excellent communication skills (oral and written);

Strong organizational skills and attention to detail, accuracy, and follow-through;

Excellent time management skills with a proven ability to meet deadlines;

Critical thinking skills;

Ability to maintain strict confidence as required by HIPAA, 42 CFR, and Oregon Statutes;

Ability to build and maintain positive and functional relationships;

Ability to function well and use good judgment in a high-paced and at times stressful environment;
