Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our Inter-Disciplinary Team as a Hospice Field Nurse. In this role, you will provide routine and emergency assessments, evaluate patient needs, and develop personalized care plans to ensure the best possible outcomes.

Key Responsibilities

  • Coordinate clinical, psycho-social, and spiritual services through case management
  • Provide physical and emotional support to patients and families during times of crisis
  • Collaborate with the Inter-Disciplinary Team to ensure continuity and quality of care
  • Explain hospice services and Medicare benefits to patients and families
  • Supervise the care provided by Licensed Practical Nurses and Hospice Aides

Requirements

  • Strong leadership and organizational skills
  • Effective communication and compassionate interpersonal skills
  • Knowledge of Microsoft Office programs
  • A valid driver's license and auto liability insurance

Education and Certifications

Registered Nursing License in the state of employment. Minimum of one year nursing practice or equivalent experience. Hospice, Oncology, Home Health, and/or Long-Term Care experience preferred.
