Job Overview

NORTH SUFFOLK COMMUNITY SERVICES INC is a leading provider of mental health services. We are seeking a highly skilled and compassionate Mental Health Outreach Coordinator to join our team.

Job Responsibilities
  • Engage individuals in the community, providing support and guidance to improve their overall well-being and self-management skills.
  • Collaborate with housing coordinators to ensure safe and affordable living arrangements for clients.
  • Develop and implement treatment plans, working closely with professional staff to address individual needs.
  • Organize and lead social and recreational activities to promote socialization and skill-building.
  • Provide medication administration and assistance as per MAP and DMH policies.
Requirements
  • Education: High School Diploma/GED equivalent required; Bachelor's Degree highly preferred.
  • Experience: At least one year of experience working with individuals with severe and persistent mental illness and/or substance use disorder.
  • Skills: MAP and CPR/First Aid certification required within five months of hire; Massachusetts Driver's License required.
