The ideal candidate will be board certified with Family Practice and/or VA experience, and will possess excellent communication skills and a passion for delivering exceptional patient care. In this role, you will be responsible for providing care to approximately 12-20 outpatients daily, with the potential for up to 25% travel required to Community Based Outpatient Clinic locations. Key Responsibilities:
- Provide high-quality patient care to a diverse population of Veterans
- Collaborate with interdisciplinary teams to develop and implement comprehensive care plans
- Participate in quality management reviews and performance improvement initiatives
- Stay current with evidence-based practices and guidelines in Primary Care
- Communicate effectively with patients, families, and healthcare teams
