Job Opportunity: We are seeking a skilled Physical Therapist to join our Home Health team at LHC Group. As a Physical Therapist, you will work with patients in their homes to provide one-on-one care and treatment.

In this role, you will be responsible for assessing and evaluating patient care needs, developing and implementing treatment plans, and providing ongoing therapy to help patients achieve their goals.

Responsibilities:

* Evaluate patient's functional status and physical therapy needs
* Assist physician in developing therapy plan of care
* Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice
* Instruct and inform participating members of the healthcare team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals

Licensure Requirements:

* Current Physical Therapy licensure in state of practice
* Current CPR certification
* Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
