Job Summary

We are seeking a skilled and empathetic Hospice Registered Nurse Case Manager to join our team. As a key member of our interdisciplinary team, you will provide direct patient care services, empower patient caregivers, and collaborate with healthcare professionals to ensure the best possible outcomes for our patients.

Responsibilities
  • Provide direct patient care services, including assessing patient needs, developing and implementing care plans, and evaluating patient progress.
  • Act as a liaison between physicians, patients, and families, ensuring seamless communication and coordination of care.
  • Empower patient caregivers by educating them on care for their loved ones, promoting independence and self-advocacy.
  • Collaborate with an interdisciplinary team, including LPNs, CNAs, and support staff, to provide comprehensive care and support.
  • Maintain accurate and detailed patient clinical records, ensuring compliance with regulatory requirements.
  • Participate in quality assessment and performance improvement initiatives, contributing to the continuous improvement of our services.
Requirements
  • RN license in the state of employment (or eligibility to obtain).
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology.
  • Additional experience in hospital staff RN, med-surg, PACU, float pool, ER, or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing is preferred.
  • Current driver's license and automobile insurance.
  • Current CPR certification.
