Job SummaryWe are seeking a skilled and empathetic Hospice Registered Nurse Case Manager to join our team. As a key member of our interdisciplinary team, you will provide direct patient care services, empower patient caregivers, and collaborate with healthcare professionals to ensure the best possible outcomes for our patients.
Responsibilities- Provide direct patient care services, including assessing patient needs, developing and implementing care plans, and evaluating patient progress.
- Act as a liaison between physicians, patients, and families, ensuring seamless communication and coordination of care.
- Empower patient caregivers through education and support, helping them navigate the complexities of hospice care.
- Collaborate with interdisciplinary teams to develop and implement comprehensive care plans, ensuring the best possible outcomes for patients.
- Maintain accurate and detailed patient records, ensuring compliance with regulatory requirements.
- RN license in the state of employment (or eligibility to obtain).
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology.
- Additional experience in hospital staff RN, med surg, PACU, float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing is preferred.
- Current driver's license and automobile insurance.
- Current CPR certification.
