Job Title: Physical Therapy Assistant

Alternate Solutions Health Network LLC is seeking a skilled Physical Therapy Assistant to join our team in Fort Thomas, Kentucky.

Job Summary:

We are dedicated to providing high-quality patient care and are looking for a motivated and compassionate Physical Therapy Assistant to work with our interdisciplinary team.

Responsibilities:
  • Provide home-based therapy for patients as directed by the attending physician and by following the order sets and goals created by the Physical Therapist.
  • Assess functional problems and provide intervention and, in conjunction with the Physical Therapist, work toward goals to improve patient functional status.
  • Counsel patient and family on the disease/injury and how to manage in all phases of physical therapy.
  • Instruct patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
  • Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
Requirements:
  • Must be a licensed Physical Therapy Assistant.
  • Must have experience working in a home health care setting.
  • Must be able to work independently and as part of a team.
  • Must have excellent communication and interpersonal skills.
