Job Title: Physical Therapist Assistant Home Health

We are seeking a skilled Physical Therapist Assistant to join our team at Alternate Solutions Health Network LLC. As a Physical Therapist Assistant, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

About the Role:
  • Provide skilled physical therapy services as ordered by the attending physician
  • Assess patient conditions and notify the Physical Therapist of any changes
  • Develop and implement individualized plans of care to improve patient outcomes
  • Collaborate with the Physical Therapist to ensure seamless care transitions
Responsibilities:
  • Plan of Care: Develop and implement individualized plans of care to improve patient outcomes
  • Patient Outcomes: Treat patients to relieve pain, develop or restore function, and maintain maximum performance
  • Educator/Advocate: Counsel patients and families on disease management and proper use of equipment
  • Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines
Requirements:
  • Physical Therapist Assistant (PTA) with current license in the state of employment
  • Minimum one-year experience as a PTA in an acute care setting
  • Valid driver's license and auto insurance in your name as a driver
