Job Title: Licensed Practical Nurse

Responsibilities:
  • Conduct thorough patient assessments and develop individualized care plans under the supervision of a Registered Nurse (RN).
  • Administer medications, treatments, and therapies as prescribed by the physician, ensuring accurate and timely delivery.
  • Monitor patient progress and report any changes in condition to the supervising RN or physician, facilitating prompt interventions.
  • Provide patient and family education on healthcare management, medication administration, and self-care techniques, promoting independence and empowerment.
  • Collaborate with the interdisciplinary team to ensure coordinated and comprehensive care delivery, addressing the unique needs of each patient.
  • Maintain accurate and up-to-date patient documentation in accordance with regulatory standards and organizational policies, ensuring seamless communication and continuity of care.
  • Provide emotional support and advocacy for patients and their families throughout the care continuum, fostering a positive and supportive environment.
Qualifications:
  • Current and unrestricted LPN license in the state of Pennsylvania.
  • Minimum of six months of clinical experience, preferably in a home healthcare or pediatric setting.
  • Strong clinical assessment and critical thinking skills, with the ability to analyze complex patient data and develop effective care plans.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work independently and collaboratively within a multidisciplinary team, prioritizing patient needs and ensuring seamless care delivery.
  • Reliable transportation and willingness to travel to patient homes within the Lansdale area, ensuring timely and efficient care delivery.
