Key Responsibilities

The LSW Director of Social Services will be responsible for:

  • Overseeing the work of the Social Service Designee
  • Helping residents and families adjust and solve problems regarding their social, emotional, and financial needs
  • Assisting with Room and Roommate Changes
  • Attending Medicare Meetings
  • Discharge planning for patients by arranging for follow up community services, home health referrals and alternative placements,
  • Advocating for residents and families regarding resident rights issues.
  • Conducting in-service training to educate staff about the emotional & social needs of residents, and offer methods for better serving residents and families
  • Attending administrative staff meetings, morning reports, Performance Improvement meetings, resident care-related meetings, and quarterly staff meetings
  • Maintaining departmental records and documentation as required
  • Coordinating and updating resident care conference schedule
  • Assisting residents and families with Medicaid approval and reapplication
  • Providing Medicaid Pending and Patient Liability related follow-up

The salary range for this role is $50,000 - $67,500 per year, depending on experience.
