Advanced Nursing Practitioner Lead

Catholic Medical Center is seeking an experienced Advanced Nursing Practitioner to join our team.
The successful candidate will be responsible for delivering high-quality patient care to clients/patients of all ages and their families within the Mobile Community Health Team (MCHT) / Health Care for the Homeless Program (HCH).
Key Responsibilities:- Deliver patient care in a manner appropriate to demographics and/or specific needs of the patient population served, ensuring excellent clinical outcomes and patient satisfaction.
- Demonstrate expertise in assessing, responding, and adapting treatment of care to meet patient population needs, promoting a culture of excellence and teamwork.
- Transport self to and from off-site locations, adhering to safety protocols and best practices.
- Lifts, carries, pushes, pulls, or moves objects up to 50 pounds occasionally, 20 pounds frequently and constantly, maintaining a safe work environment.
- Demonstrate courtesy, compassion, and respect when relating to client/patients, families, visitors, physicians, and co-workers, fostering a positive working relationship.
- Communicate effectively and establish a cooperative working environment, promoting clear communication and collaboration.
- Facilitates communication between others, resolving conflicts and improving relationships.
- Accept constructive feedback and initiate actions to correct situations, embracing continuous improvement and growth.
- Identify and take action toward meeting own learning needs, enhancing professional development and skills.
- Endure heavy workloads or stress, prioritizing tasks and managing time effectively.
- Graduate of an accredited APRN program, demonstrating a strong foundation in advanced nursing practice.
- Two years as a mid-level provider, with a proven track record of success and growth.
- Experience or interest in caring for homeless or poverty medicine population settings preferred, highlighting commitment to underserved populations.
- Advanced Cardiovascular Life Support (ACLS) certification preferred, demonstrating expertise in cardiovascular life support.
- Licensed by the state of NH as an Adult practice APRN, adhering to state regulations and requirements.
