Key Responsibilities
  • Clinical Leadership: Provide medical direction and oversight to our primary care team, ensuring high-quality patient care and outcomes.
  • Panel Management: Manage a panel of patients, providing regular check-ins, health coaching, and care coordination to ensure optimal health outcomes.
  • Team Development: Lead and develop our primary care team, providing coaching, mentoring, and performance management to ensure team members are equipped to deliver exceptional patient care.
  • Operational Excellence: Collaborate with our operations team to drive process improvements, streamline workflows, and enhance patient flow.
  • Quality Improvement: Develop and implement quality improvement initiatives to enhance patient satisfaction, reduce readmissions, and improve clinical outcomes.
  • Communication: Serve as a liaison between our primary care team, hospitalists, specialists, and other healthcare providers to ensure seamless care coordination and communication.
  • Training and Development: Participate in ongoing training and development opportunities to stay current with best practices, new technologies, and emerging trends in primary care.
Requirements
  • Medical Degree: Must possess a medical degree (MD or DO) and be board certified in Internal Medicine, Family Medicine, Geriatrics, or a related specialty.
  • Clinical Experience: Minimum 2 years of clinical experience in geriatric, adult, or family practice setting.
  • Leadership Experience: Minimum 2 years of clinical leadership experience, preferably in a primary care setting.
  • Communication Skills: Excellent communication and interpersonal skills, with the ability to effectively communicate with patients, families, and healthcare teams.
  • Technical Skills: Proficient in Microsoft Office Suite and other relevant software applications.
