As a Palliative Care RN Case Coordinator, your responsibilities will include:
- Serving as the primary contact among the physician, patient, family, and hospice team
- Delivering direct nursing care to hospice patients while offering emotional support to their families
- Educating caregivers on effective care techniques for their loved ones
- Collaborating with a multidisciplinary team to ensure comfort, peace, and dignity for patients
- Assessing the needs of patients and families to assist in developing and implementing care plans
- Maintaining accurate clinical records for all assigned patients
- Engaging in quality assessment and performance improvement initiatives
Qualifications for the Palliative Care RN Case Coordinator:
- Valid RN license in the state of employment (or eligibility to obtain)
- Minimum of 1 year of nursing experience in hospice, home health, ICU, geriatrics, or oncology preferred
- Current driver's license and automobile insurance
- Current CPR certification
