**Job Summary:**
We are seeking a skilled Registered Nurse to join our team as a Patient Flow Coordinator, responsible for ensuring seamless admission, discharge, and transfer processes for our patients.
Key Responsibilities:
- Conduct thorough admission assessments on patients within 24 hours of admission.
- Identify and address barriers to discharge for patients with estimated discharge dates.
- Collaborate with interdisciplinary teams to optimize patient flow and reduce length of stay.
- Develop and implement strategies to improve patient satisfaction and experience.
Requirements:
- Graduate of an accredited nursing program (Associate's Degree, Diploma, or BSN).
- Current RN licensure in the state of hire and/or compact licensure.
- Basic Life Support certification through the American Heart Association or equivalent.
