The Licensed Practical Nurse (LPN) (Certified) plays a crucial role in delivering personalized nursing care to elderly individuals and those facing acute health challenges. Under the supervision of a Registered Nurse (RN), the LPN utilizes both technical skills and clinical knowledge to cater to the unique needs of residents and their families. This position is essential in promoting health, preventing illness, and ensuring comfort for all patients.
Key Responsibilities
- Deliver direct patient care and assist with medical procedures as directed, ensuring adherence to established protocols and competencies. Collaborate with the RN for assessments and recommendations, participating actively in patient evaluations and care planning.
- Manage and implement physician orders, ensuring accurate communication with the healthcare team and timely documentation of care provided.
- Engage in medication management, ensuring the safe administration of prescribed medications and monitoring patient responses.
- Complete clinical documentation promptly, maintaining compliance with regulatory standards and company policies.
- Participate in the clinical admission process for new residents, ensuring a smooth transition into care.
- Support the clinical discharge process, assisting residents and families with necessary follow-up care.
- Contribute to ongoing quality improvement initiatives by participating in audits and implementing recommendations.
Qualifications
- Graduate from an accredited LPN School of Nursing.
- Possession of a valid LPN license in the state of employment.
- Current CPR certification and certification in specialized clinical areas such as long-term care, gerontology, wound care, IV therapy, or hospice care.
- Preferred experience of six months in a long-term care setting.
Skills and Abilities
- Ability to respond effectively in emergency situations.
- Strong clinical skills with a commitment to high-quality patient care.
- Excellent communication skills to interact with residents, families, and healthcare team members.
