Job Title: Licensed Practical Nurse/Licensed Vocational Nurse

Job Summary:

The Licensed Practical Nurse/Licensed Vocational Nurse is a vital member of our healthcare team, providing direct skilled client care under the supervision of a registered nurse. This role requires a strong commitment to delivering high-quality patient care, working collaboratively with the team to achieve positive client outcomes.

Key Responsibilities:

  1. Provide direct client care as defined under the scope of nursing practice and legislative acts, utilizing sound judgment and critical thinking.
  2. Implement the plan of care and treatment as required, ensuring accurate and timely documentation.
  3. Conduct ongoing assessments, providing accurate and detailed information to the registered nurse.
  4. Participate in health/therapeutic counseling, teaching, and emotional support to clients and their families/caregivers.
  5. Observe client condition and accurately report to the registered nurse, ensuring prompt intervention when necessary.
  6. Assist with client discharge or transfer, ensuring a smooth transition.
  7. Assess and provide client and family/caregiver education and information pertinent to diagnosis.
  8. Prepare, administer, and document actions taken specific to commonly prescribed oral, topical, subcutaneous, and intramuscular medications, observing and reporting adverse reactions to medications and/or treatment to the registered nurse.
  9. Recognize the need for and institute emergency measures in a rapid and appropriate manner, when indicated.
  10. Participate in coordination of home health services, reporting identified needs for other disciplines to the Care Designer/Director of Nursing or registered nurse supervisor.
  11. Perform all skills, treatments, and procedures competently in accordance with State Board of Nursing scope of practice and adhere to the policies, procedures, and protocols of the agency.
  12. Follow principles of infection control and Universal Precautions.
  13. Identify and participate in performance improvement activities.
  14. Use equipment properly and in a safe manner.
  15. Ensure client's rights are adhered to, including but not limited to:
    1. The right to privacy and dignity while care is being provided.
    2. The right to accept or refuse care.
    3. The right to confidentiality of their information.

Qualifications:

  • Graduate of an accredited licensed practical nurse program from a community college.
  • Current Certificate of Competence and in good standing with the State Board of Nursing.
  • 1 year of nursing experience required; three years nursing experience preferred. Community, home health, or medical/surgical experience preferred.
  • Comply with regulations and legislation.
  • Comply with accepted professional standards of practice.
  • High level of interpersonal skills.
  • Certified in CPR.
  • Strong clinical skills.
  • Excellent customer service skills and ability to deal tactfully with clients, other team members, and community.
  • Treat clients, team members, and the public with courtesy, respect, and present a positive public image.
  • Empathy for the needs of the client.
  • Works as a team member.
  • Self-directing with the ability to work with little direct supervision.
  • Licensed driver with automobile that is insured in accordance with the organization's requirements.
  • Negative TB test or chest x-ray.

Environment, Personal Protective Equipment, or Physical Demands:

Contact with clients under a wide variety of circumstances. Care provided in client's living environment with varying situations or in an institution such as a hospital. May be required to respond to emergency situations. Travel required. Position includes tasks that have the potential for exposure to blood and other potentially infectious material (OPIM) such as body fluids/tissues.

May include gloves, mask, eye/face protection, and disposable outer covering or lab coat.

Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull. Varied assistance is required depending on the ability of the client. Must be able to lift up to 50 pounds. Visual and hearing acuity required.
