Job Summary

We are seeking a skilled Physical Therapist Assistant to join our team at Interim Healthcare of West Michigan. As a Physical Therapist Assistant, you will play a vital role in providing high-quality patient care and contributing to the success of our post-acute care services.

Key Responsibilities
  • Participate in ongoing interdisciplinary assessments of patients to develop and implement effective care plans.
  • Collaborate with physical therapists and other healthcare professionals to provide comprehensive patient care.
  • Document patient progress and respond to treatment in accordance with professional standards and payor requirements.
  • Counsel and educate patients and their families on healthcare-related needs and promote patient progress toward mutually established goals.
  • Communicate effectively with other members of the healthcare team to ensure coordination of patient care and planning for discharge.
Requirements
  • Current licensure as a physical therapist assistant in the state of Michigan.
  • At least two years of experience as a physical therapist assistant in the last five years.
  • Active CPR certification.
  • Previous home health experience is a plus.
