Key Responsibilities- Interdisciplinary Assessment: Participate in ongoing interdisciplinary assessment of patients to ensure comprehensive care.
- Plan of Care: Act at the direction of the treating physical therapist and in accordance with the plan of care to provide effective patient treatment.
- Documentation: Complete and submit timely, legible, accurate, relevant, and sufficient documentation addressing treatment and interventions provided, patient response, and progress toward goals.
- Counseling and Education: Counsel and educate patients and their families to promote patient progress and meet healthcare-related needs.
- Communication: Effectively communicate with other members of the healthcare team to promote coordination of patient care and planning for discharge.
- Licensure: Current licensure as a physical therapist assistant in the state of Michigan.
- Experience: Two years of experience as a physical therapist assistant in the last five years.
- Certification: Active CPR certification.
- Previous Experience: Previous home health experience is a plus.
