Healthcare Advocate I

LifeLong Medical Care is a dynamic organization dedicated to providing innovative healthcare and social services to a diverse patient community. We are seeking a skilled Healthcare Advocate I to join our team in delivering high-quality patient care.
Job Summary
The Healthcare Advocate I will be responsible for providing services to patients with complex care needs. This includes conducting patient outreach, engagement, and psychosocial service assessment, as well as developing and implementing patient-centered care plans. The successful candidate will be the lead implementer of Enhanced Care Management (ECM) and will coordinate service referrals and delivery.
Responsibilities
Key responsibilities of the Healthcare Advocate I include:
- Providing patient outreach and engagement services through telephone and in-person interactions at LifeLong, community, and residential sites
- Assessing patient strengths and needs through the use of standard intake and screening tools, as well as health and social services records review
- Actively involving patients and caregivers in the design and delivery of services, including the development of care plans that align with patients' values and goals
- Providing and facilitating referrals for internal and external resources, and collaborating with patients to complete required applications and forms
- Maintaining a patient caseload in accordance with LifeLong standards for the specific population served or site requirements
- Utilizing data registries and reports to manage caseload, meet program requirements, and promote high-quality care
- Providing health education and training to patients on topics such as harm reduction and disease risk-mitigation strategies
- Assisting patients with accessing and retaining public benefits and insurance, as well as affordable housing
- Respectfully and routinely communicating with patients, their care team members, external partners, and social supports
- Maintaining knowledge of patients' medical and behavioral health treatment plans and facilitating service utilization
- Participating in team meetings to coordinate care, support patient goals, and reduce barriers to accessing services
- Advocating on behalf of patients to get their needs met and/or supporting patients to learn advocacy strategies
- Keeping current on community resources and social service supports to effectively serve the target population
Requirements
The ideal candidate will have a commitment to working directly with low-income individuals from diverse backgrounds in a culturally responsive manner. They will also possess strong organizational, administrative, and problem-solving skills, as well as excellent interpersonal, verbal, and written skills. Additionally, the candidate will be able to prioritize tasks, work under pressure, and complete assignments in a timely manner.
Qualifications
Key qualifications for the Healthcare Advocate I include:
- High School diploma or GED
- At least one year of work or equivalent volunteer experience in a community-based healthcare or social service setting
- Previous work or volunteer experience providing services to individuals with complex health and/or social services needs
- Proficient skills using Microsoft Office applications, as well as the ability to work in and/or manage databases
- Access to reliable transportation with current license and insurance
Salary and Benefits
The estimated salary for this position is $26.39/hour.
