Nutrition Specialist

Responsibilities
- Assist patients in making informed diet and nutrition choices to improve their quality of life
- Conduct comprehensive nutrition assessments for new and at-risk patients
- Collaborate with the interdisciplinary team to develop and implement individualized care plans based on nutritional assessments
- Identify specific interventions necessary to meet nutritional needs, based on evidence-based guidelines and best practices
- Provide education and support to patients and their families on renal diet and fluid management
- Monitor laboratory results and weight changes, making recommendations to meet established goals
- Utilize a patient-centered care approach, respecting individual and cultural preferences while encouraging engagement
- Participate in Quality Assessment Performance Improvement processes, documenting and attending meetings as required
Qualifications
- Excellent communication skills
- Demonstrated clinical proficiency
- Ability to problem solve and collaborate with care teams
Education/Training:
- Must be a Registered Dietitian Nutritionist with Commission on Dietetic Registration
- Minimum one year professional work experience in clinical nutrition as a registered dietitian required
- Previous renal care experience preferred, but not required
