Acute Care Clinical Pharmacist

Job Summary
The Pharmacist provides high-quality pharmaceutical care to patients in an acute care setting. This involves ensuring the provision of safe, effective, and cost-effective medication therapy for all patients.
The Pharmacist will develop, recommend, and monitor evidence-based pharmacotherapy care plans for patients cared for by Geisinger, with a focus on patient outcomes and quality of life.
These care plans include assessing patients' medications for indication, effectiveness, safety, and adherence issues, as well as patient and laboratory parameter monitoring, documentation requirements, and outcomes.
Job Duties
The Pharmacist assumes responsibility for delivering pharmaceutical care to the specific patient care area in which they are scheduled to work.
They coordinate the use of human, material, and informational resources to ensure timely, efficient, cost-effective, and appropriate care is consistently rendered.
The Pharmacist receives and interprets physicians' medication orders and clarifies or corrects orders with the physician.
They review patient medication profiles to guard against drug sensitivities, contraindications, duplications, and major drug interactions.
The Pharmacist reviews all medication orders for prescribing, formulary, and therapeutic issues.
They suggest appropriate formulary alternative(s) based on the patient's clinical parameters.
The Pharmacist consults and advises members of the medical staff on matters relating to specific drug therapy problems and monitors and acts on patient laboratory results.
They provide in-depth drug information and other informational materials to members of the medical, nursing, and other healthcare professionals.
The Pharmacist provides patient discharge medication consultations based on department standards.
They make rounds with medical services.
The Pharmacist calculates drug dosages based on pharmacokinetic parameters.
They verify provider-entered orders and ensure proper doses dispensed.
The Pharmacist provides in-services to other healthcare providers when requested and when necessary.
They prepare special medications involving compounding procedures and sterile products using aseptic techniques.
The Pharmacist reports requirements for additional supplies and equipment to appropriate personnel to maintain basic stock levels.
Leadership and Collaboration
The Pharmacist coordinates and supervises workplace activities and resources, promotes professional competence, and ensures that services rendered and work performed are in accordance with departmental standards.
They advise, assist, and supervise the work of pharmacy technicians, students, and pharmacy interns, assuming final responsibility for the accuracy of their work.
The Pharmacist works with medical and nursing personnel to resolve problems involving pharmacy services.
They coordinate activities with other pharmacy personnel to accomplish pharmacy and hospital objectives.
The Pharmacist assists with the review and revision of departmental policies and procedures, as requested.
Requirements
The Pharmacist must have a Bachelor's Degree in Pharmacy and a minimum of 6 years of related work experience.
They must be licensed as a Pharmacist in the State of Pennsylvania within 90 days of employment.
