About You: To be successful in this role, you should have a current, unencumbered license to practice nursing as an RN/LPN in Florida. A minimum of two years of RAI and Care Plan experience, preferably in a Skilled Nursing Facility (SNF), is also required. Your knowledge of MDS 3.0, including Assessments, CAA's, and Care Plans, will enable you to conduct each Assessment and Care Plan with the appropriate participation of health professionals. Additionally, your ability to organize work procedures, assume responsibility, and tactfully interact with other disciplines will be valuable assets in this position.

Responsibilities: As a Healthcare Nurse Specialist, you will be responsible for assigning responsibility for completing sections of the MDS to staff who have clinical knowledge about the patient. You will also ensure that non-licensed staff who provide direct care to the patients are allowed to contribute information for the assessment process. Furthermore, you will conduct, complete, or coordinate other disciplines in completion of the RAI and certify the completion of the Assessment. You will sign and certify the CAA Summary Form, attend care planning, and enter the care plan decision information. Additionally, you will assist with month-end duties for closeout and perform other assignments as assigned.
