Job DescriptionLongevity Health is seeking an Advanced Practice Provider to join our team. As a key member of our care management team, you will be responsible for providing high-quality, patient-centered care to our nursing home residents.

Key Responsibilities:

* Provide collaborative primary care and services to a panel of Longevity Health members
* Execute and deliver Longevity Health Model of Care activities
* Facilitate and administer Health Plan clinical operations functions
* Serve as a trusted advisor to members, loved ones, and nursing home staff
* Deliver effective and frequent communication and coordination with members' PCPs, loved ones, and care team members

Requirements:

* State unrestricted licensed Advanced Practice Nurse or Physician's Assistant
* National Board certification as an Adult, Family, Geriatric, or Primary Care nurse practitioner or Physician's Assistant
* DEA License
* Active national provider number (NPI) and active Medicare provider number
* Valid state driver's license with a good driving record and proof of automobile insurance

Preferred Qualifications:

* Experience working in the nursing home environment
* Computer/Technology proficiency
* Excellent people skills
* Self-directed but functions well in a team environment
* Excellent clinical skills - assessment, diagnosis, treatment, prescribing specifically for the chronically ill population
