This role is a Per Diem position focused on Home Visits for Hospice patients. Previous experience in Home Health or Hospice is essential.

In your role as the RN Case Manager, you will take full nursing responsibility for implementing the Plan of Care for each hospice patient, while continuously assessing both personal and professional performance and making necessary adjustments to enhance productivity and quality of care.

  • Conduct the initial nursing assessment to determine eligibility for hospice services during a visit within forty-eight (48) hours of referral.
  • Identify the physical, psychosocial, and environmental needs of the patient and family, reassessing as necessary, at least every fifteen (15) days.
  • Initiate and coordinate the care plan while maintaining precise and relevant clinical documentation regarding the patient's condition.
  • Record issues, set appropriate goals, interventions, and document patient/family responses to hospice care.
  • Collaborate daily with the patient/family, attending physician, and other members of the interdisciplinary group to provide comprehensive care.

Experience Requirements

  • A minimum of one year of clinical experience.

License Requirements

  • Current RN licensure in the state of practice along with one year of clinical experience.
  • Current CPR Certification.
  • A valid driver's license, valid vehicle insurance, and access to reliable transportation or public transit.
