Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Hospice RN Case Manager. In this role, you will provide high-quality patient care, coordinate with healthcare teams, and support patients and their families through the hospice care process.

Responsibilities
  • Provide direct patient care services to hospice patients and supportive care to patients' families
  • Act as primary liaison between physician, patient/family, and hospice team
  • Manage and collaborate with interdisciplinary teams to provide comfort, peace, and dignity
  • Evaluate patient and family needs and assist in preparing and executing the plan of care
  • Maintain all assigned patient clinical records
  • Participate in company quality assessment and performance improvement programs
Requirements
  • RN license in the state of requested employment (or eligibility to obtain)
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
  • Current drivers license and automobile insurance
  • Current CPR certification
