We are seeking a skilled Occupational Therapist to join our team at Allaire Rehab & Nursing. As an Occupational Therapist, you will play a vital role in improving the daily living skills and independence of our residents in a skilled nursing facility.
Key Responsibilities- Assessment and Evaluation: Conduct comprehensive assessments of residents' functional abilities, focusing on activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
- Individualized Care Plans: Develop tailored intervention plans to enhance residents' independence in daily activities.
- Section GG Data Collection: Ensure accurate and consistent collection of Section GG data for Annual, Quarterly, Falls, and Significant Change assessments.
- Treatment: Administer OT interventions, including ADL training, adaptive equipment provision, and environmental modifications.
- Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conduct remote check-ins through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care.
- Documentation: Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs (Net Health/PCC).
- Interdisciplinary Collaboration: Collaborate with other rehab staff, including Physical Therapists and Speech-Language Pathologists, for holistic resident care.
- State licensure as a Physical Therapist.
