About the Role

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Community Options, Inc. in Hillsborough, NJ. As a LPN, you will play a vital role in delivering high-quality care to individuals with disabilities in a community setting.

Key Responsibilities
  • Provide comprehensive nursing care to individuals, including administering medications, monitoring health conditions, and developing individualized care plans.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Conduct regular health assessments, identify potential health risks, and develop strategies to mitigate them.
  • Train staff on medical procedures, protocols, and best practices to ensure high-quality care.
  • Monitor and maintain accurate records of individual health information, including medical histories, treatment plans, and progress notes.
  • Participate in community-based activities, social events, and recreational programs to promote socialization and engagement.
  • Develop and maintain relationships with community members, businesses, and families to build a support network for individuals.
Requirements
  • Bachelor's Degree or equivalent experience in nursing field.
  • Minimum of one year of experience providing nursing services to individuals with disabilities.
  • Valid nursing license and valid driver's license with a satisfactory driving record.
  • Ability to calculate figures related to medication dosages and exercise good judgment in crisis situations.
  • Excellent verbal and written communication skills, with the ability to effectively present facts and recommendations.
  • Proficient with Microsoft Office and ability to work effectively in a team environment.
