Job Responsibilities

As an LPN with IntelyCare, you will be responsible for delivering post-acute care to residents, monitoring their physical and mental wellbeing, and tracking their vital signs. You will also perform intakes of residents, document their health concerns and medical histories, and review their medical records and charts. Additionally, you will properly administer and verify medications, help maintain appropriate levels of medical supplies, and contribute to a safe and comfortable environment for residents.

Qualifications and Skills
  • Current Pennsylvania State License: Must be licensed to practice as a Licensed Practical Nurse (LPN) in Pennsylvania
  • Education: Completion of an approved Licensed Practical Nurse (LPN) educational program
  • Experience: Minimum of 6 months of experience as an LPN or in a related role
  • Physical Demands: Ability to lift a minimum of 50 lbs
