Home Risk Assessment Nurse Practitioner

As a Nurse Practitioner with Matrix Medical Network, you will have the opportunity to make a direct impact on the health and well-being of individuals in need. Our comprehensive health assessments provide unmatched insight into a patient's overall health and well-being, allowing us to potentially close multiple care gaps with a single visit.
Responsibilities- Conduct thorough adult/geriatric assessments, medical history reviews, diagnoses, and treatment plans, as well as health education and physician referrals
- Collaborate with physicians, case managers, social workers, family members, caregivers, and other healthcare professionals as needed
- Provide services in various settings, including home visits, skilled facility visits, and telehealth visits
To be successful in this role, you must possess:
- A Master's Degree or equivalent experience and satisfactory completion of NP licensure
- Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain it
- Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty
- A current BLS, ACLS, or CPR certification
- A minimum of 1 year of experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health records software
- Excellent verbal and written communication skills
- The ability to travel and drive a car with adequate insurance coverage
This is a PRN/Part-Time position with flexible scheduling, offering a competitive hourly rate of $90.00 per completed assessment.
