Home Health Risk Assessor

We are seeking a skilled Nurse Practitioner to join our team as a Home Health Risk Assessor. This is a part-time PRN position, offering flexible scheduling and the opportunity to make a direct impact on the health and well-being of others.
About the RoleIn this role, you will conduct comprehensive health assessments in patients' homes, reviewing medical history, medication adherence, social environment, and other risks. You will provide unmatched insight into a patient's overall health and well-being, helping to improve quality of care and potentially close multiple care gaps with a single visit.
Responsibilities- Conduct home-based assessments, medical history reviews, diagnoses, and treatment plans
- Collaborate with physicians, case managers, social workers, family members, and caregivers as needed
- Provide education to patients and their families on healthy lifestyle choices and disease management
- Work with primary care physicians to ensure continuity of care
- Document all patient interactions and assessments according to Matrix guidelines
- Master's degree in Nursing required or equivalent experience and satisfactory completion of NP licensure
- Current RN and NP licensure in the state of practice, including prescription authority or ability to obtain
- Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty
- Current BLS, ACLS, or CPR certification
- Minimum 1 year experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health record software
- Excellent verbal and written communication skills
- $90.00 per completed assessment
- Flexible scheduling, including days, evenings, nights, and weekends
