Job Summary

Key Responsibilities
  • Provide patient-centered care, prioritizing the needs of patients and families
  • Develop and implement individualized plans of care, ensuring multidisciplinary planning and consultation
  • Monitor patient progress, revising plans as needed to ensure optimal outcomes
  • Collaborate with the healthcare team to ensure seamless care transitions and effective communication
  • Participate in performance improvement activities, identifying opportunities for growth and improvement
Requirements
  • Minimum six (6) months of experience in an acute or sub-acute facility within the last three (3) years
  • Registered Nurse License (California)
  • Basic Life Support from American Heart Association
