LVN Charge Nurse

We are seeking a skilled and compassionate LVN Charge Nurse to join our team at Windsor Nursing and Rehabilitation Center of Alice in Alice, TX.
Job Summary:The LVN Charge Nurse will play a vital role in ensuring the delivery of high-quality care to our residents. This includes assessing resident needs, developing and implementing individualized care plans, and collaborating with the healthcare team to achieve optimal outcomes.
Key Responsibilities:- Conduct regular rounds to assess resident condition and identify areas for improvement
- Develop and implement individualized care plans in collaboration with the healthcare team
- Administer medications as prescribed and monitor resident response
- Collaborate with the physician to ensure accurate and timely documentation of resident care
- Oversee and monitor the work of nursing aides to ensure high-quality care
- Initiate and complete incident reports as necessary
- Discharge residents in accordance with facility policy
- Active Licensed Vocational Nurse in the state of Texas in good standing
- Experience working in a skilled nursing facility preferred
- Ability to effectively communicate with residents, families, and healthcare team members
The physical demands of this role include:
- Lifting up to 50 pounds
- Carrying up to 50 pounds
- Pushing/pulling up to 50 pounds
- Standing, sitting, walking, and balancing
- Speaking and hearing
