Key Responsibilities:
- Conduct comprehensive assessments of patients' needs, including OASIS assessments.
- Develop and implement individualized treatment plans in consultation with physicians.
- Collaborate with the interdisciplinary team to ensure seamless care delivery.
- Teach, supervise, and counsel patients and their families on physical therapy programs.
- Supervise Physical Therapist Assistants and communicate effectively with other healthcare professionals.
- Document patient progress and communicate changes to physicians and other team members.
Requirements:
- Graduate of an accredited Physical Therapy program.
- Current licensure to practice as a PT in the State of Florida.
- Basic Life Support (BLS) certification.
- Valid driver's license and reliable transportation.
- At least one year of experience as a Physical Therapist, with home healthcare experience preferred.
