Responsibilities
  • Evaluate patients' functional status and physical therapy needs, and develop personalized treatment plans to improve their mobility, endurance, and overall well-being.
  • Provide one-on-one therapy sessions to patients in their homes, using evidence-based techniques and modalities to promote optimal recovery and function.
  • Collaborate with other healthcare professionals, including nurses, physicians, and social workers, to ensure comprehensive and coordinated care.
  • Document patient progress and outcomes, and communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Current Physical Therapy licensure in the state of Florida.
  • Current CPR certification.
  • Must have a valid driver's license and reliable transportation.
