Job Summary

We are seeking a skilled Healthcare Coordinator to join our team at Trinity Health. As a key member of our care management team, you will play a vital role in supporting the care coordination activities of our care team.

Key Responsibilities:

  • Mail educational materials and letters to patients
  • Schedule patient appointments and locate outside patient records
  • Collaborate with community partners to provide comprehensive care
  • Use general knowledge of medical conditions, nutrition, and diet to educate patients
  • Assist in locating and updating patient clinical records
  • Facilitate patient and caregiver access to community resources
  • Screen patients for depression, substance use, and mental health disorders

Requirements:

  • High school diploma
  • Medical Assistant Certificate preferred
  • Qualified as a Certified Application Counselor for MassHealth program
  • Minimum of five years' work experience in a healthcare setting
  • Minimum of two years' experience within a healthcare related organization
