Role Summary

The Continuing Care Liaison plays a crucial role in promoting our healthcare services, including home health, hospice, rehabilitation, and physician services, within the community. This position involves conducting educational sessions and collaborating with healthcare providers, facilities, and community organizations to enhance awareness of our continuum of care services.

Qualifications
  • A Bachelor's degree or higher in a relevant field, or equivalent professional experience.
  • Understanding of hospice and home healthcare philosophies, Medicare regulations, and state laws pertaining to these services.
  • Experience in healthcare, particularly in medical sales and marketing, is preferred.
  • Strong written, verbal, and public speaking abilities.
  • A valid driver's license and a clean driving record are required.
Key Responsibilities
  • Build and maintain effective relationships with referral sources, including physicians, hospital case managers, nursing home staff, and other healthcare professionals.
