Job Summary:

The LPN - ER Care Continuum Specialist will play a critical role in coordinating the continuum of care for patients referred to the Emergency Department. This includes facilitating communication with ED physicians and charge nurses, obtaining and updating patient documentation, and ensuring a seamless interaction with the ED and other disciplines/units/facilities.

Key Responsibilities:
  • Coordinate the continuum of care for patients referred to the ED
  • Facilitate communication with ED physicians and charge nurses
  • Obtain and update patient documentation
  • Ensure a seamless interaction with the ED and other disciplines/units/facilities
  • Collaborate with transfer facilities, charge nurses, and house supervisors to facilitate admissions
  • Obtain verbal orders as necessary
  • Follow up on lab and radiology results and consult with physicians as needed
  • Assist with pharmacy call backs and other ED calls that require chart review
  • Coordinate plan of care with patients, families, and interdisciplinary teams
Requirements:
  • LPN licensure in the state of Texas
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment
  • Strong problem-solving and critical thinking skills
  • Ability to maintain confidentiality and handle sensitive information
