Key Responsibilities:

  • Develop and implement individualized plans to meet the unique needs of each participant.
  • Provide personalized support and care to ensure participants' physical and emotional well-being.
  • Collaborate with colleagues to maintain a safe and inclusive environment.
  • Communicate effectively with participants, family members, and healthcare professionals.
  • Document progress and outcomes to inform future planning and service delivery.
