Community Living Transition Specialist

Springwell is excited to introduce a new initiative aimed at assisting individuals of all ages in their transition from Long-Term Care facilities to community living arrangements.
ROLE SUMMARY:
The Community Transitions Program Liaison will:
- Engage with residents of skilled nursing facilities to promote awareness of our services and programs.
- Identify suitable candidates for the program based on their interest in moving to a community living environment.
- Facilitate individualized planning and assessments through collaborative interdisciplinary team meetings.
- Serve as a primary contact for residents, families, nursing facility staff, and other stakeholders involved in the care and transition process.
- Manage referrals to appropriate programs and ensure follow-up to support successful community discharges.
- Collaborate with various state programs and teams to assist individuals in their transition, including screening for community waiver programs.
REQUIREMENTS:
- A Bachelor's Degree or higher is preferred.
- Applicants lacking a Bachelor's Degree may be considered if they possess:
- An Associate's Degree along with a minimum of 3 years of experience in elder care or case management; OR
- At least 50% of the coursework required for a Bachelor's Degree, with a commitment to complete the remaining coursework within three years.
- Familiarity with long-term care, case management, discharge planning, and community resources to facilitate successful transitions.
- Strong customer service skills and experience working with diverse populations.
- A reliable vehicle, valid driver's license, and a clean driving record are essential.
- Proficiency in computer applications, including Microsoft Word, Excel, and Outlook.
