Social Services Specialist

Job Summary:
St. Claire Regional Medical Center is seeking a skilled Social Worker to join our team. As a Social Worker, you will play a vital role in identifying and addressing the psychosocial needs of our patients. You will work closely with our interdisciplinary team to develop and implement effective discharge plans, ensuring a safe and timely transition to the next level of care.
Key Responsibilities:
Conduct thorough psycho-social assessments to identify patient needs
Collaborate with care teams to develop and implement discharge plans
Provide social services to patients, including education and resource guidance
Identify and resolve problems that impede discharge planning or patient access to healthcare resources
Maintain accurate records and documentation of patient interactions and discharge plans
Requirements:
Bachelor's Degree in Social Work required; Master's Degree preferred
Two years of clinical experience in acute care social work, care management, and discharge planning
Must become licensed in Kentucky within 18 months of employment
