The Mount Sinai Health System is seeking a skilled Physician to join our team in Post-Acute Care at the Mount Sinai Health System in New York City. As a Physician in this role, you will have the opportunity to provide high-quality patient care and work in a collaborative environment with a team of healthcare professionals.
Key Responsibilities- Completes comprehensive evaluations of new patients and provides effective management of chronic medical conditions.
- Works collaboratively with the interdisciplinary team to develop and implement patient care plans.
- Ensures seamless care transitions through direct communication with community, facility, or hospital staff.
- Participates in performance improvement activities and contributes to the development of clinical metrics.
- Collaborates with other healthcare professionals to provide education and training to medical students and residents.
- Medical degree from an accredited university.
- New York State medical license.
- Board certification in Internal Medicine or Family Medicine.
- Additional certification in Geriatrics and/or Palliative Medicine is preferred.
- Competitive salary range from $180,000 to $280,000.
